C-CHANGE PEARLS Clinical Practice Guidelines Workshop

Session Description
It is anticipated that after the session participants will be able to take away ‘practice pearls’ and implement the C-CHANGE cardiovascular harmonized guidelines in their practice to optimize screening, diagnosis and management of patients with multiple morbidities. Participants will engage in interactive small group discussions using case studies led by facilitators. Q & A sessions will follow each case study including local experts: Physicians, Nurses and Pharmacists.

 

Objectives

Upon completion of this case study, participants should be able to:

​

Module:  CV Risk Reduction in Diabetes (SGLT2 inhibitors/GLP-1 Receptor Agonists in Patients with DM and CVD)

  • Describe the patient who should be treated with an SGLT2i or GLP-1 agonist.

  • Explain the rationale for this treatment and the potential benefits.

  • Describe the steps for initiating and monitoring therapy for patients with diabetes.
     

Module: Hypertension and Stroke Prevention and Management in Patients
Upon completion of this case study, participants should be able to:
​

  • Plan to assess and screen patient’s blood pressure at all appropriate visits.

  • Discuss modifiable cardiovascular risk factors with patients.  

  • Discuss blood pressure targets and need for anticoagulation/anti-platelet therapy in people who have just suffered a stroke.  

​

Module: How C-CHANGE can help clinicians manage the multi co-morbidity patient

  • Identify recommendations for multiple morbidities from the C-CHANGE guidelines.

  • Implement recommendations for multiple morbidities in a single patient.

  • Use the C-CHANGE recommendations to help keep the elderly living at home longer and healthier.

Follow up and Evaluations 

Pre and post workshop assessments will be administered to the participants (to generate learning needs, goals of attendees and baseline knowledge and practices).  After the workshop, at 3-6 months, participants will be sent a follow up survey questionnaire to determine whether any changes have occurred.  For family physicians seeking study credits, the program will include practice reflection exercises to be approved, under the new MAINPRO+ accreditation standards.

Dr. Sheldon Tobe MScCH (HPTE), FRCPC, FACP, FASH is a hypertension specialist and nephrologist with a research background in clinical outcomes trials, and knowledge translation and implementation of clinical practice guidelines. He is currently Professor in Medicine at the University of Toronto where he is a staff physician in the Division of Nephrology and also Professor of medicine at the Northern Ontario School of Medicine. Dr. Tobe is active in guidelines development and is a co-chair of the Canadian Cardiovascular Harmonized National Guidelines Endeavour (C-CHANGE) guidelines as well as the Hypertension Canada clinical practice guidelines and Diabetes Canada clinical practice guidelines.

 

Dr. Rahul Jain MD, CCFP, MScCH (HPTE) is an academic Family Physician and Hospitalist at Sunnybrook Health Sciences Centre. He is an Assistant Professor in the Department of Family and Community Medicine at the University of Toronto. Dr. Jain is actively involved in knowledge translation and implementation of clinical practice guidelines related to cardiovascular health and collaborates on a number of initiatives with C-CHANGE and Diabetes Canada. He completed his Masters in Health Practitioner Teacher Education and has been a recipient of several awards in teaching and continuing education.

 

Dr. Thor Christensen, MD, CCFP is a Primary Care Physician at Montague Health Centre, Kings Network, Health PEI. He completed his Medical Degree at Dalhousie University. Thor pursued family medicine training in becoming a chief resident in the PEI Dalhousie family medicine training program on the island. Since completing the program he’s been practicing in his home town (Montague) as a primary care physician who covers hospital on call schedule and emergency medicine. Dr. Christensen is Associate Professor of Family Medicine at Dalhousie University and Memorial University. He is active in mentoring and precepting Family Medicine Students, and in 2018 he was named PEI’s Family Medicine Preceptor of the year.

Dr. Lenley Adams, MD FRCPC FACP, Department Head, Internal Medicine, Queen Elizabeth Hospital

Medical Director, Provincial Diabetes Program, Charlottetown, Prince Edward Island

Lenley Adams received his medical degree from Dalhousie University in 1991 and completed training in Internal Medicine at Dalhousie in 1995. After practicing in Northern Ontario and Bridgewater, Nova Scotia, in August 2008, he relocated to Charlottetown where he practices Internal Medicine with a focus on diabetes care and general endocrine problems but continues to practice in ICU and cardiac care. He has been the medical director for the Provincial Diabetes Program since 2008 and was recently a co-author for the 2018 Diabetes Canada Clinical Practice Guidelines chapter on Management of Type 1 Diabetes in Adults.

Shannell MacKinnon, Nurse Practitioner, Primary Care, Montague Health Centre, Kings Network

Shannell graduated with her BScN from UPEI in 2004, and worked primarily in Emergency Nursing, with Intensive Care, Northern Outpost Nursing, and Supervisory experience as well. She returned to graduate school in 2012 at Dalhousie University, and while working full time and raising three boys, graduated with her Masters in Nursing-Nurse Practitioner Degree in 2015. Shannell has worked for three years in the provincial diabetes program and continues to have a passion for diabetes care. Over the past year she has been working as a member of the collaborative primary care team at Montaque health center. Shannell spends most of her time at various sports fields and arenas depending on the season, while maintaining a passion for her work and continuous learning.

 

Andrea Trainor-Tweel, BSc(Pharm), CDE, Community Pharmacist

Andrea has been a community pharmacist for 31 years and as a Certified Diabetes Educator for 15 years. Prior to becoming a CDE, Andrea had an encounter at her pharmacy with a gentleman, who was picking up an antibiotic for a foot sore. His concern was how quickly the medication would work as his foot was too sore to wear his work boots. Within a short period of time he was diagnosed with diabetes, had an amputation and he never returned to work.  From that experience, Andrea states, she became aware of how much diabetes affected her patients and wanted to expand her knowledge to better improve and extend their lives.

Dr. Heather Williams, MD FRCPC CSCN (EMG), BSc., Neurologist

Dr. Heather Williams is a general neurologist with an interest in stroke who is currently practicing in Charlottetown. She completed her medical school training at the Memorial University of Newfoundland and her residency training at the University of Ottawa. She has a specific interest in stroke prevention and is working with a great team in PEI to improve stroke care for Islanders. Dr. Williams originates from New Brunswick and moved to PEI in 2017. Fun Fact: In her spare time she takes care of her three greyhounds and enjoys exploring PEI.

 

Tara Roche, RN, MN, Acting Manager & Clinical Leader, Primary Care, Queens East Network, Health PEI

Tara Roche loves being a nurse and now has 19 years of varied experiences.   She graduated with her Bachelor of Science in Nursing from the University of Prince Edward Island in 2000 and in 2011 completed her Masters of Nursing at Athabasca University.  She worked in an Emergency Department in Winnipeg, taught at the University of Prince Edward Island and most recently has been managing the Queens East Primary Care Network.  She has been working in Primary Care for the past 7 years.  Tara has a passion for examining the social determinants of health and their impact on the population Primary Care partners with.

Wendy Cooke, BSc(Pharm), Clinical Pharmacist, ICU, Queen Elizabeth Hospital, PEI

Wendy Cooke has been practicing as a hospital pharmacist since her graduation from Dalhousie College of Pharmacy. Since 2007, she has been the clinical pharmacist in the ICU at the Queen Elizabeth Hospital, PEI. Wendy is part of a multidisciplinary team in the ICU, working closely with Nurses, Physicians, Respiratory Therapists and many other allied health care practitioners. She has been involved in a number of policy and practice initiatives including her work with the Provincial Parenteral Therapy Committee. One of her main areas of interest is anticoagulation therapy.

Dr. Martha Carmichael MD, MSc, FRCPC, Geriatrician, PEI Provincial Geriatric Program

Martha joined the Provincial Geriatric Program in 2017 after beginning her career at Mount Sinai Hospital in Toronto.  She completed both medical school and a residency in Internal Medicine at Dalhousie University, before completing a fellowship in Geriatric Medicine at the University of Toronto.  She previously obtained Bachelor of Science (Hons) and Master of Science degrees in Neuroscience, both at Dalhousie University.  She is presently involved in research projects in the areas of medical education and quality improvement, and has an interest in the areas of Advanced Care Planning, end-of-life issues, program development, health systems and health policy.

 

Kirsten Mallard MN NP GCN(C), Geriatric Nurse Practitioner, PEI Geriatric/Home Care Programs

Kirsten has held nursing roles in hemodialysis, acute care, and advanced care in northern nursing however the majority of her career has been in Home Care.  Working as Home Care Team Leader, she obtained her Masters of Nursing, NP stream, Family of All Ages at Dalhousie University in 2014. Following a clinical placement in geriatrics Kirsten joined the Provincial Geriatric Program and is currently working in a collaborative practice as a Geriatric Nurse Practitioner with the Geriatric/Home Care Programs. Kirsten’s clinical expertise and interpersonal skills have been instrumental in launching the COACH (Caring for Older Adults in the Community and at Home) Program recognized in 2018 as a top five frailty innovation by the Canadian Frailty Network.

 

Chris McKenna, BSc. Pharm, Community Pharmacist, Lawtons Drugs, Montague

Chris is the Pharmacy Manager at Lawtons Drugs in Montague. Lawtons is located in the Montague Medical Centre which is a multi-disciplinary site with physicians, nurse practitioners, nurses and dietitians and it is also beside the Kings County Medical Centre. He has been actively involved with the COPD and addictions programs in the area for several years and believes in collaboration with all health care professionals.
